Note (1): The above BD Standard Forms RR-9 to RR-16 and RR-10A to RR-16A can also be used by an RMWC(Co) to make the following applications:-
  -  application for Registration of Additional Class and/or Type of Minor Works for an RMWC(Co) via Specified Form BA25C
  - application for Application for Approval of Nomination of Additional Authorized Signatory (AS) / Technical Director (TD) via Specified Form BA25D.
Note (2): The English versions of the updated BD Standard Forms RR-12 to RR-16, RR-12A to RR-16A (Apr 2011 revision) are under preparation.  In the meanwhile, applicants may use:-
  -  the Chinese versions (Apr 2011 revision) of these updated forms; or
  - the previous English version (Dec 2009 issue) of Forms RR-12 to RR-15, RR-12A to RR-15A,
